It’s important to plan for your family’s financial security in case the unexpected happens, from a long-term illness to a death in the family.

 

Overview

To help protect the financial wellbeing of your loved ones, Lurie Children’s provides basic life insurance for employees — at no cost to you — along with the opportunity to purchase supplemental coverage and accidental death and dismemberment (AD&D) insurance. You also receive short-term disability by the hospital at no cost, along with long-term disability, if you have completed one year of benefits eligible service.

Be sure to name a beneficiary

It’s important to designate a beneficiary to receive the benefit paid by a life insurance policy. As personal circumstances change, take the time to keep that information up to date. Visit your Benefits' portal to add or change a beneficiary.

 

Basic Life Insurance

Lurie Children’s provides basic life insurance to assist you and your family in the event of a death or dismemberment. These benefits are fully paid by the company and coverage is automatic — you do not need to enroll.

Supplemental Life Insurance

If you want more financial protection beyond the basic coverage that Lurie Children’s provides, you can purchase supplemental life insurance for yourself and your family members. You pay the full cost of this coverage, which is provided through The Hartford.

Additional life insurance coverage:

  • Yourself: Elect an additional one to three times your annual salary
  • Spouse: $5,000, $10,000 or $25,000 of coverage
  • Child: $2,500, $5,000 or $10,000 of coverage
Personal Health Application

As a newly eligible employee, you can enroll for employee life insurance up to $300,000 without proof of your health status. If you elect coverage over $300,000, you will be prompted to complete a personal health application (PHA). PHA may be required if you choose to increase your employee or spouse coverage at a later date. If your election requires PHA, the Benefits' portal will open a link to The Hartford website for you or your spouse to complete an online PHA. You will be notified if your PHA is not approved.

 

Accidental Death & Dismemberment

Accidental Death & Dismemberment (AD&D) insurance provides coverage if you were to die or lose the use of a limb in an accident. You can choose coverage for yourself and your family members. Coverage for the Unexpected

Should you lose your life, sight, hearing, speech, or use of your limb(s) in an accident, AD&D insurance provides additional benefits to help keep your family financially secure. AD&D benefits are paid as a percentage of your coverage amount — from 50% to 100% — depending on the type of loss.

Short-Term Disability

Short-Term Disability (STD) provides you with pay in the event you are unable to work due to illness or a non-work related accident. You are eligible for STD coverage when you reach six months of service.

  • Coverage is automatic and company-paid; no need to enroll.
  • Benefits begin on the 8th calendar day of a disability, if hospitalized greater than 23 hours, benefits begin day 1.
  • Pays 60% of your budgeted weekly salary.
  • Benefits end after 26 weeks.

Long-Term Disability

Long-Term Disability (LTD) provides coverage to ensure that at least a part of your income continues until you can return to work due to an illness or a non-work related accident. You are eligible for LTD after completing one year of benefits eligible service.

  • Coverage is automatic and company-paid; no need to enroll.
  • Benefits begin after you have been disabled for 180 days.
  • Pays 60% of your salary.

Voluntary LTD

  • You may choose to elect and pay for an additional 6-2/3% monthly benefits for a total benefit coverage of 66-2/3%.
